During addiction treatment and rehabilitation, the bond of companionship service dogs can provide, might help ease the anxiety of getting treatment.<sup>1</sup> Service dogs are welcome on a case-by-case basis at American Addiction Centers’ treatment facilities.

Service dogs perform essential actions and tasks for people with disabilities. Additionally, service dogs must be able to comply with commands in public spaces and in distracting environments.<sup>2</sup>
<h2><strong>Can Service Dogs Be Taken Anywhere?</strong></h2>
Under the ADA (Americans with Disabilities Act), service dogs are allowed to go everywhere their owner goes, including healthcare facilities, doctor’s offices, and patient’s rooms.<sup>2</sup> Through aiding in the navigation and support of daily tasks, service dogs provide their owners independence. Some of the tasks service dogs perform include:<sup>3</sup>
<ul>
 	<li>Guiding people with visual impairments.</li>
 	<li>Signaling people who are deaf or have hearing impairments.</li>
 	<li>Retrieving items for people with mobility issues.</li>
 	<li>Alerting people during life-threatening conditions, such as cardiac episodes or seizures.</li>
</ul>
Service dogs provide companionship that can boost their owner’s self-esteem and motivation for self-improvement.<sup>3</sup>
<h2>Emotional Support Dogs</h2>

Going to rehab can be a very stressful time for those suffering from mental health disorders, which is why American Addiction Centers consider allowing emotional support dogs to accompany their owners through treatment.

Emotional support animals (ESAs) are animals (mostly dogs) that provide their owners, who have been diagnosed with a mental health disorder, with emotional support and companionship.<sup>4</sup> Some mental health disorders—like <a href="https://americanaddictioncenters.org/anxiety-and-addiction" target="_blank" rel="noopener">extreme anxiety disorders</a>, <a href="https://americanaddictioncenters.org/co-occurring-disorders/ptsd-addiction" target="_blank" rel="noopener">PTSD</a>, <a href="https://americanaddictioncenters.org/bipolar-and-addiction" target="_blank" rel="noopener">bipolar disorder</a>, and <a href="https://americanaddictioncenters.org/treating-depression-substance-abuse" target="_blank" rel="noopener">depression</a>—can make daily tasks extremely difficult without the support of an ESA.

Unlike service dogs, emotional support canines do not need to attend obedience training or be able to perform any specific tasks to be qualified as an ESA. However, they do have to behave in public, which means they don’t cause disturbances.<sup>5</sup> Furthermore, while some states or local governments permit individuals to take ESAs in public spaces like restaurants and grocery stores; others may have restrictions against it.<sup>6</sup> Check with your state and local government agencies to understand the laws in your area. However, the Fair Housing Act mandates that landlords permit ESAs even if there’s a “no pet policy” without additional pet fees.<sup>7</sup>
<h2><strong>How to Get a Service or Emotional Support Dog</strong></h2>
<h3><strong>Service Dog</strong></h3>
First, your healthcare provider needs to provide written documentation that you are being treated for a physical disability, debilitating chronic illness, or neurological disorder that affects one or more limbs.<sup>7 </sup>The severity of the condition determines service dog eligibility. For instance, an individual with epilepsy must have at least one seizure per month to qualify for a service dog.<sup>8</sup> However, doctors can help accommodate exceptions in certain situations.

Then you’ll need to find a service dog provider, agencies that train service dogs specifically for the people who need them, including:
<ul>
 	<li>Veterans.</li>
 	<li>The blind.</li>
 	<li>Individuals with other physical impairments.</li>
</ul>
These agencies then put the trained dogs up for adoption for those with physical and mental disabilities. Call or visit the agency to ask about the training process and the disability training the for which they specialize. After you find the right fit, submit your application.
<h3><strong>Emotional Support Animal</strong></h3>
You may already own the animal you’d like to use as your EAS, or you can adopt or get one. To legitimately make them and ESA, you’ll need a letter from your licensed professional mental health professional stating that you have a mental health condition, such as depression or severe anxiety that limits one or more major life activities—like the ability to sleep, work, learn, or travel—and that the ESA helps alleviate the symptoms associated with the mental illness or emotional distress.<sup>9 </sup>The letter needs to include:<sup>10</sup>
<ul>
 	<li>The mental health provider’s signature and date, and this should be on their letterhead.</li>
 	<li>The mental health professional’s license type, number, and state where it was issued.</li>
 	<li>Documentation that you have a mental health disorder.</li>
 	<li>A recommendation for an EAS to help alleviate your symptoms.</li>
</ul>
There’s no need to register or certify the animal. This letter provides appropriate documentation for airlines (though it’s only valid for one year) and landlords.
<h2><strong>How Dogs Can Help in Substance Abuse Treatment</strong></h2>
While research on human-animal interactions is still relatively new, studies indicate that when humans interact with animals, cortisol (a stress-related hormone) levels decrease, blood pressure lowers, feelings of loneliness reduce, and feelings of social support and overall mood boost.<sup>11</sup>

One study found that adding shelter-dog recreational activities to an adolescent <a href="https://americanaddictioncenters.org/rehab-guide/inpatient">inpatient substance use disorder program</a> enhanced the effects of interventions.<sup>12</sup>

Another study’s findings suggest that when used as a part of <a href="https://americanaddictioncenters.org/therapy-treatment/cognitive-behavioral-therapy">cognitive-behavioral therapy</a>—a therapy favored by many treatment programs—dogs can make emotionally evocative subjects less distressing and may even help bring about positive outcomes during the process.<sup>13</sup>

Having a routine is a great thing to keep your mind focused and clear. Taking breaks throughout your workday, eating healthy and balanced meals, getting exercise and fresh air daily, and avoid binge watching hours of TV, are just a few ways to stay positive and healthy.

<h2>Healthy Activities to Do at Home</h2>

What to Do During COVID-19 Isolation

Ambien was initially promoted as a safer alternative to <a href="https://americanaddictioncenters.org/benzodiazepine">benzodiazepines</a>, with less risk for addiction or tolerance, but studies show that this has been underestimated.<sup>1</sup> There are also serious health risks associated with long-term use of Ambien, including a potentially increased likelihood of respiratory issues, reflux, and infections.<sup>3, 4 </sup>

Ambien works similarly to benzodiazepines but is slightly less likely to make you feel groggy the next day or experience dependence.<sup>1,5</sup>



Other sedative-hypnotics include:<sup>1</sup>

<h2>Ambien Risks and Addiction</h2>
Risks and side effects are always something to aware of with medications and other drugs. The potential for misuse, tolerance, physiological dependence, and withdrawal with Ambien is higher than initially thought.<sup>1,6</sup> Side effects can include:<sup>3,6,7</sup>
<ul>
 	<li>Aggression.</li>
 	<li>Anxiety.</li>
 	<li>Depression.</li>
 	<li>Doing things you can’t remember.</li>
 	<li>Drowsiness.</li>
 	<li>Impaired driving.</li>
 	<li>Lack of coordination.</li>
 	<li>Nightmares.</li>
 	<li>Slowed reaction time.</li>
 	<li>Suicidal thoughts.</li>
</ul>
People with insomnia find it difficult to fall or stay asleep. If you have been using Ambien long-term and have tried to stop using the drug, your insomnia can worsen.<sup>1</sup>

Some additional long-term risks may include: worsening sleep apnea, reflux, chronic sinus infections, coughs, and laryngitis.<sup>6</sup>

Since you can develop tolerance to the effects of Ambien, you may find yourself taking larger doses to get the same results from the drug.<sup>1 </sup>Instead of feeling sleepy, some people report feeling euphoric when they take Ambien.<sup>8</sup>



If you are taking Ambien and have concerns, or <a href="https://americanaddictioncenters.org/rehab-guide/addiction-guide-for-family-members">if a loved one has an addiction to the drug</a>, you should be aware of these potential signs of abuse:<sup>1,3,9</sup>
<ul>
 	<li>Buying Ambien illegally.</li>
 	<li>Combining it with other substances.</li>
 	<li>Driving under the influence of Ambien.</li>
 	<li>Experiencing withdrawal symptoms when trying to stop.</li>
 	<li>Forging prescriptions.</li>
 	<li>Taking it differently than prescribed.</li>
 	<li>Sleeping through important events because of Ambien.</li>
 	<li>Strong cravings.</li>
 	<li>Trying to cut down or stop without success.</li>
 	<li>Using Ambien every night.</li>
 	<li>Visiting multiple doctors for prescriptions.</li>
</ul>

<h2>Ambien Withdrawal and Treatment</h2>
If you have been taking Ambien in high doses or for a long time, you may experience more severe withdrawal symptoms.<sup>6</sup> Withdrawal may start within 48 hours of stopping or cutting back on Ambien use.<sup>6</sup> Symptoms of withdrawal can include:<sup>1,2,3,6</sup>

 	<li>Anxiety or panic attacks.</li>
 	<li>Delirium.</li>
 	<li>Disorientation.</li>
 	<li>Hallucinations.</li>
 	<li>Increased blood pressure, pulse, breathing rate, or body temperature.</li>
 	<li>Sweating.</li>
 	<li>Fatigue.</li>
 	<li>Insomnia.</li>
 	<li>Irritability.</li>
 	<li>Nausea or vomiting.</li>
 	<li>Restlessness.</li>
 	<li>Seizures.</li>
 	<li>Stomach cramps and abdominal discomfort.</li>
 	<li>Tremors.</li>
</ul>
Medically managed withdrawal, also known as medical detox, is when you decrease the use of a substance in a facility while under the care of trained medical staff.<sup>10</sup> Case report evidence supports there being a potential risk of withdrawal seizures. For this reason, medical management of Ambien withdrawal could be beneficial as this is potentially a life-threatening complication.<sup>3,6,10</sup> Staff at a treatment or detox facility can provide medication and constant monitoring to ensure your safety.



After completing detox, you will begin treatment. You should discuss your options with your treatment team to decide what is right for you. Treatment options can include:<sup>11</sup>
<ul>
 	<li><a href="https://americanaddictioncenters.org/rehab-guide/inpatient"><em>Inpatient treatment</em></a><strong>:</strong> This is a facility where you stay at and receive intensive group and individual treatment and therapy. For patients recovering from Ambien use, you will learn about addiction, relapse prevention, self-care, and how to manage insomnia without resorting to addictive sleep aids.</li>
 	<li><a href="https://americanaddictioncenters.org/rehab-guide/outpatient-treatment"><em>Outpatient treatment</em></a><strong>:</strong> This is a facility where you attend several times a week for group and individual sessions, and a psychiatrist session monthly while living at home. This allows you to benefit from similar care as an inpatient setting, although it is relatively less intensive and less restrictive. Outpatient treatment is best for people who have completed inpatient treatment or have strong, supportive social networks at home.</li>
</ul>
<h2>Addiction Recovery</h2>
Recovery from an Ambien addiction means learning to live sober. Practicing techniques to manage symptoms of insomnia can help you stay away from sleeping pills. For someone with unmanaged insomnia, recovery will also involve learning about your diagnosis, what triggers your symptoms, and how to apply coping skills.

The National Institute on Drug Abuse (NIDA), in 2018, reported that 58.5% of teens, by their senior year in high school, had consumed alcohol and 47% had use illicit drugs. Drug abuse is also seen in college students in a 2018 survey from NIDA, showed that young adults ages 19-22 used the following substances:<sup>2</sup>

 	<li>Marijuana use was about 43% in collage students, which is approximately a 7% increase over 5-years.</li>
 	<li>Vaping with nicotine products were used 15.5% for college students and 12.5% for non-collages adults.</li>
 	<li>Drug use varied on the type of substance:

 	<li>Prescription opioid use among college students dropped to 2.7% and among non-college adults dropped to 3.2% since 2013.</li>
 	<li>Adderall abuse occurred in 14.6% among college men and 8.8% among college women; in non-college men 5.3% and non-college women 10.1%.</li>

 	<li>Binge drinking occurred to 28% among college students and 25% among non-college adults.</li>
</ul>

<h2>Why Do I Need Same Day Detox?</h2>
Potential situations where quick admission to a <a href="https://americanaddictioncenters.org/drug-detox">detox program</a> may help avert dangerous consequences (if a person is already experiencing significant symptoms or another emergency, call 911):

Additionally, same day admission to an "emergency" detox program for you or a loved one might be beneficial if a patient is experiencing or is at risk of experiencing <a href="https://americanaddictioncenters.org/withdrawal-timelines-treatments/opiate">opioid withdrawal symptoms</a>. Abstinence from opioids can decrease tolerance, so a smaller amount of opioids might cause an overdose in a relapse, which could be fatal.<sup>4 </sup>If an individual pursuing addiction treatment warrants pharmacological treatment but maintenance treatment is impractical, is not indicated, or is turned down, then detoxification is indicated.<sup>1</sup>

Better results correlate with methadone or buprenorphine treatment that is ongoing than with medically supervised withdrawal. Relapse to opioid use will occur, research indicates, for the majority of individuals who have opioid use disorder and go through medically supervised withdrawal, though psychosocial strategies can be beneficial during and after. After an adequate amount of time without using opioids, injectable extended-release naltrexone can be used, which is better at lowering the likelihood of using opioids again than no medicine or placebo.<sup>5</sup>

<h2>What is the Detox Process?</h2>
According to a consensus panel, there are three crucial elements in the detox process:<sup>1</sup>
<ol>
 	<li><strong><em>“Evaluation”</em></strong>: This involves testing for substances of abuse, checking for co-occurring health problems (physical and psychological), assessing any health problems present, and evaluating the individual's social circumstances.</li>
 	<li><strong><em>“Stabilization”</em></strong>: This includes helping a person to get through any current state of intoxication and withdrawal. Medical and/or psychosocial interventions may be used. Also included is helping individuals become acquainted with their part in their addiction treatment and their recovery as well as with what they should anticipate in the treatment environment.</li>
 	<li><em><strong>“Fostering the patient’s entry into treatment”</strong>:</em> This component includes, through emphasizing that continuing on with the whole treatment continuum is important, getting an individual ready to enter addiction treatment programs. Patients with a history of repeatedly detoxing but not participating in substance abuse treatment after might be motivated to do so by signing a treatment contract early, once sufficiently stable.</li>

<h2>Intensity of Detox Care</h2>
There are several factors to consider when determining the appropriate care for your, or a loved ones detoxification, for example:<sup>1</sup>

 	<li><em>Obtaining a substance abuse history</em>: This may include questions about what substances the patient abused, how often the patient used them, what amount(s) the patient used, and how long it has been since the patient last used them.</li>
 	<li><em>Getting information on any prior detoxes and/or treatments for substance abuse</em>: Whether the patient has gone through withdrawal before and, if so, how many times and what happened, ought to be part of this.</li>
 	<li><em>Laboratory tests</em>: Determining blood alcohol content (BAC), checking breath alcohol levels, and/or urine drug screening may be part of the evaluation. Lab tests may also be used to assess nutritional status and to screen for health problems.</li>
 	<li><em>Physical assessment</em>: Through a physical exam, the patient can be assessed for any health concerns.</li>
 	<li><em>Gathering a</em> <em>history concerning overall health</em>: Questions may include inquiring if the patient has any mental health conditions, any physical health conditions, and/or any allergies, as well as if the patient has had any surgeries and/or has ever had seizures.</li>
 	<li><em>Assessing the person’s current living situation</em>: Assessing living situation may include asking if the patient is homeless, stays in a shelter, or lives with others.</li>

<h2>How Long Does Detox Treatment Take?</h2>
The length of stay for an inpatient detox program may vary based on the substance used and your individual experiences and needs. Detox can last anywhere from a few days (3, 5, 7, 10 day detox) to a few weeks depending on the substance. Once stabilized through the detox process, patients will typically enter an <a href="https://americanaddictioncenters.org/rehab-guide/inpatient">inpatient rehab program</a> at the same facility where they will attend therapy and learn techniques for long term recovery.
